Rehabilitation Coordinator – Gloucestershire County Council Location: Great Western Court, Gloucester 
Contract: 6-Month Temporary (2 positions available) 
Pay Rate: £20.05 per hour 
Hours: 37.5 per week (variable shift pattern across 7 days) 
Shifts: Early (07:30–15:30) or Late (15:30–22:00) 
About the Role We’re recruiting on behalf of 
Gloucestershire County Council for two 
Rehabilitation Coordinators to join the team at 
Great Western Court — a service supporting individuals through their rehabilitation and care programmes following hospital discharge or community admission.
 You’ll lead shifts within a multi-disciplinary team, ensuring residents receive high-quality care and rehabilitation support. This role involves administering medication, implementing therapy-led goals, and promoting independence in a positive and caring environment. 
Key Responsibilities- Lead shifts and coordinate rehabilitation programmes for individuals
- Administer, prompt, or assist with medication safely and accurately
- Support residents with personal care and mobility in line with care plans and risk assessments
- Work closely with the therapy and care teams to review and update plans
- Maintain detailed written and electronic care records
- Manage admissions and discharges smoothly, encouraging independence and dignity
- Supervise Rehabilitation Assistants and act as Officer in Charge when required
Requirements✅ Enhanced DBS (Adults Barred List)
 ✅ Level 3 qualification in Health & Social Care (or equivalent experience)
 ✅ Experience administering medication and supporting therapy-led rehabilitation
 ✅ A positive, practical, and team-focused approach
 ✅ Excellent communication and organisational skills
