About the Role:
We are currently recruiting for an experienced Occupational Therapist to join the Adults Services Team in Powys.
This role focuses on delivering high-quality, person-centred Occupational Therapy interventions to support adults in maintaining their independence and wellbeing. You’ll be working as part of a multidisciplinary team, assessing needs, and recommending tailored solutions to help service users live safely and independently within their homes and communities.
Key Responsibilities:

• Complete functional assessments and develop outcome-focused care plans
• Provide specialist advice on equipment, adaptations, and moving and handling
• Work collaboratively with service users, carers, and other professionals
• Support safe hospital discharges and community-based reablement
• Maintain accurate and timely case records

Requirements:

• Qualified Occupational Therapist
• Registered with the Health and Care Professions Council (HCPC)
• Previous experience working within Adult Social Care
• Strong assessment and communication skills
• Knowledge of relevant legislation and safeguarding
