👩‍⚕️ Qualified Social Worker – Adult Social Care
📍 Location: North East | 💷 £25 per hour

💼 Key Responsibilities
In this role, you will:

Conduct comprehensive assessments of adults’ care and support needs
Coordinate safe and timely hospital discharges in partnership with NHS colleagues
Manage a varied caseload with autonomy and professionalism
Develop individualised care plans that promote independence, dignity, and empowerment
Work in line with current legislation and best practice in adult social care
Build trusted relationships with service users, carers, and multi-agency teams
Offer support and guidance to unqualified staff and students, helping to build capability across the team

We're looking for someone who is:

A Qualified Social Worker registered with Social Work England
Experienced in adult social care, particularly in hospital discharge or community settings
Skilled in risk assessment, care planning, and multi-disciplinary collaboration
Knowledgeable in relevant legislation and frameworks
A clear communicator, problem solver, and advocate for vulnerable adults
Proactive, compassionate, and committed to achieving positive, sustainable outcomes

Minimum: 4+ years post-qualification experience preferred.
