About the Role:
We are seeking an enthusiastic and dedicated Social Worker to join an adults locality team in the North East a temporary contract basis. You will play a key role in supporting adults with care and support needs to live as independently as possible, promoting choice, wellbeing, and personal dignity in line with the principles of the Care Act 2014.

The Localities Team works with adults of all ages, including older people, people with physical disabilities, and those with mental health or learning support needs. You will work closely with individuals, families, carers, and partner agencies to deliver high-quality, person-centred social work practice within the local community.

Key Responsibilities:

• Undertake assessments and reviews in line with statutory duties and local procedures.
• Develop and implement personalised care and support plans that promote independence and wellbeing.
• Work collaboratively with health professionals, housing, voluntary organisations, and other partners.
• Safeguard adults at risk by identifying and managing concerns in accordance with safeguarding procedures.
• Support service users to access community resources and support networks.
• Maintain accurate and timely records using electronic case management systems.
• Contribute to multi-disciplinary meetings and team discussions.

What you need:

• A degree in Social Work or equivalent
• Registered with Social Work England
• Completed relevant post-qualifying training
• Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, and relevant legislation and guidance.
• Experience of working with adults in a social care or health setting
• A full UK driving licence and access to a vehicle (where required).
• Enhanced DBS check
• Excellent communication and written skills
